services that set community standards and exceed customer' expectations in a convenient, cost-effective and accessible manner.

Defining Health Services:

Purpose of Nursing Facilities

Long term care refers to a continuum of medical and social services designed to support the needs of people living with chronic health problems that affect their ability to perform everyday activities. Long-term care services include traditional medical services, social services, and housing. The goals of long term care are much more complicated and considerably more difficult to measure than the goals of acute medical care.

STNA/CNA

Home health aides provide assistance to the chronically ill, the elderly, and family caregivers who need relief from the stress of caregiving. Many home health aides also work in assisted living facilities, which provide independent living in a homelike group environment, with professional care available as needed. As advances in medicine and technology extend the lives of people with chronic illnesses, the number of people needing health care will increase. The need for home health aides will also increase.

LPN

Home Health Care LPN is a highly qualified health care provider who through education and experience possesses a distinct body of knowledge and skills. He/she applies that knowledge and skills in assessing and evaluating client response to actual or potential health problems. The Home Health Care LPN promotes and restores clients’ health by providing skilled care and collaborating with HOC Supervisors, co-workers and other medical professionals engaged in the care of the client.

RN

A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisors.
